Overview

LIJ Valley Stream was selected by Northwell Health for Accountable Care Unit (ACU® care model) implementation within their system. The unit selected for the initial implementation was a 22-bed med-surg unit, with one geographically co-located hospitalist team.

Objectives

The unit was hoping that the patient-centered care processes would be advantageous to their patient population which had a high percentage of non-English speaking families in the community.

Methods

1Unit provided online and onsite training to unit staff over a number of days, that also included hospital leaders (Chief Medical Officer, Chief Nursing Officer and Chief Executive Officer).

The onsite training led to robust conversations between frontline unit staff and hospital leaders about how the ACU care model was going to work on the unit. The resulting outcomes of going through these conversations resulted in a phenomenal Accountable Care Unit that achieved amazing outcomes.

The hospital system reported 12 month pre- and post-implementation results for the unit.

Cost outcomes

Nursing overtime variance to budget: $72,000 Pre-SIBR vs. $15,000 Post-SIBR ($57,000 reduction)

Conclusion

Implementation of the ACU care model with SIBR® rounds on the 22-bed med-surg unit resulted in significant clinical and cost improvements. The unit was awarded a Northwell Health Quality Improvement Award in 2017. This success resulted in spread of the model and processes to additional units at the hospital and other hospitals.
