Overview

Hospitalists at BayCare Health Morton Plant had heard about the first Accountable Care Unit at Emory University Hospital and were keen to adopt and implement the model on their own hospitalist unit to see if the results emerging from EUH could be replicated.

Objectives

To evaluate the impact of the Accountable Care Unit (ACU care model) on unit outcomes on a hospitalist unit.

Methods

The physician lead for the 30-bed hospitalist unit visited the ACU at Emory University Hospital and had regular implementation support calls with the EUH leadership team during launch.

The unit reported 12 months pre- and 8 months post- implementation results.

Results

Length of Stay

  1. 16% reduction in length of stay

  2. 25% reduction in 30-day readmissions

Conclusion

Implementation of the ACU care model showed signiigcant improvements in throughput and satisfaction outcomes on the 30-bed hospitalist unit.
